Membership Pricing and Services
Ages 60 Years +: $110/month
Ages 41 – 59 Years: $95/month
Ages 18 – 40 Years: $75/month
Ages 5-17 Years: $55/month
Family ( up to 4 ): $250/month
With your membership, there are no co-pays or surprise fees. You get direct access to your physician, longer office visits (30-60 minutes) and same/next day scheduling. We also offer heavily discounted labs. We will make every effort to keep the cost of meeting your health needs down without sacrificing excellent, high quality, evidenced based medical care.
Your membership services include:
Comprehensive Primary Care
- Annual physical exams including well woman and well child exams, sports and camp physicals.
- Management of chronic conditions such as diabetes, high blood pressure, asthma and depression.
- Contraception management (including Nexplanon and IUDs
- Pre-op evaluations
- Acute care visits including: testing and management of COVID, flu, strep throat, urinary tract infections, sexually transmitted infections, pregnancy testing.
- Care coordination with your specialty and hospital care.
- House calls as appropriate.
In-Office Procedures
- EKGs (coming soon)
- Cryotherapy of warts, and other non-cancerous skin lesions. (coming soon)
- Skin lesion removal and biopsy*
- Simple laceration repair and incision
- Nebulizer treatments.
- Pap smears
- Colposcopy (coming soon)
- Endometrial biopsies*
- IUD and nexplanon removal/insertions*
*cost of pathology for biopsies and contraceptive device is not included
If your membership lapses, there is a $75 charge for re-enrollment.
